Its here in Florida. I think the name of the company is US Body Source. Search for it by that name and I think you'll find it.

I think Ralph Voorhees [ club member] might be able to guide you on this. I know he bought a front end from a company in Tenn. but I do not know their name. By the Way, Do we have another race truck in the works??????? Please fill us in......

I picked up #718/2280 (95) from dr10016. Truck is totaled sold all but cab,bed,fram and doors. I,m thinking of a pro street.Thanks for the input!

Yup it is US Body Source. The front end is $995. You'll have to remove all items from the fender wells and some fabrication and critical measurements will have to be done by the installer. Looks like a cool piece though.
I think the place in Tennessse is called "Creative Glas Works". And they are currently not making the fiberglass bumpers for our trucks.
